Passport applications are accepted for processing Monday – Friday between the hours of 8:30 am – 3:00 pm. By Appointment
Applicants need the following upon arrival to made application:
DS-11 Form – Completed in black ink. single sided, no whiteout.
All applicants, including minor children, must apply in person.
It is required that any minor child under the age of 16 must be accompanied by both parents at the time of application. Parents will need to provide valid Photo ID
Long form birth certificate which includes both parent’s names and a raised seal by state Vital Records Department where birth occurred, OR a passport book or card expired more than 15 years.

Applicant photograph – taken within the last 6 months, no camouflage or glasses.
Passport Renewal
You are able to renew your passport on your own and save the $35.00 processing fee by completing and sending the DS-82 Form directly to Passport Services, If the following applies:
You are able to submit your most recent passport book or card.
Your previous passport book or card was issued on or after your 16th birthday, and your most recent passport book or card was issued less than 15 years ago.
Most recent passport was issued in your current name or you can document your name change.
Our office does not process Form DS-82.
